Dit boek is vooral bestemd voor zorgverstrekkers en andere mensen die professioneelactief zijn in de gezondheidssector. Het boek heeft twee doelstellingen. Ten eerste wil het een positieve visie overbrengen omtrent de rol van het economisch denken in de gezondheidszorg. Economie staat te veel synomiem met besparen en financiële druk.Economie gaat echter over het optimaal besteden van de beschikbare financiële middelen. Een goede toepassing van het economisch denken in de gezondheidszorg houdt in dat men de gezondheidsector als een productieve sector beschouwt, met als doel gezondheid te produceren, door ervoor te zorgen dat mensen langer en gezonder leven. Maar wie zegt "productief" zegt ook "productiviteit": de maatschappij moet met de beschikbare middelen trachten zoveel mogelijk aan gezondheid te winnen, en daarom moet men voorrang geven aan interventies (preventief of curatief) die pergeïnvesteerde euro het meest gezondheid opleveren. De methode die gehanteerdwordt bij het maken van dergelijke keuzes, is de gezondheidseconomische evaluatie. En dat brengt ons bij de tweede doelstelling van het boek, met name deze methodevan gezondheidseconomische evaluatie uitdiepen en uitleggen. Steeds meer worden niet-economen geconfronteerd met resultaten van dergelijke evaluaties, bv. in medische tijdschriften. Maar ze slaan die artikels vaak over, want onbekend is onbemind. Gezondheidseconomie gaat over het verstandig investeren in gezondheid en gezondheidszorg. Het doel van dit boek is de lezer vertrouwd te maken met de principes, methodes én valkuilen van gezondheidseconomische evaluaties. Deze evaluaties helpen om keuzes te maken en prioriteit te geven aan die investeringen (voor preventie, diagnostiek, genezing of zorg) die toelaten de grootste hoeveelheid gezondheid te winnen per geïnvesteerde eenheid van geld.De resultaten van zulke evaluaties vindt men steeds vaker terug in wetenschappelijke tijdschriften, op congressen en in beleidsbeslissingen. Dit boek helpt iedereen die betrokken is bij gezondheid en gezondheidszorg om de methode van deze evaluaties te begrijpen en hun resultaten te interpreteren. In deze tweede editie, met talrijke recente voorbeelden, is er aandacht voor relatief nieuwe methoden (onder andere om kwaliteit van leven te meten) en voor de onvermijdelijke trends (onder meer het gebruik van data uit de reële praktijk). The question of how to generate sufficient revenue to pay for health care has become a serious concern for nearly all European policy-makers. This book examines the advantages and disadvantages of funding arrangements currently in use across Europe. Adopting a cross-national, cross-disciplinary perspective, it assesses the relative merits of the main methods of raising resources including taxation; social, voluntary and supplemental forms of insurance; and self-pay including co-payments. The health systems we enjoy today, and expected medical advances in the future, will be difficult to finance from public resources without major reforms. Public health spending in OECD countries has grown rapidly over most of the last half century. These spending increases have contributed to important progress in population health: for example, life expectancy at birth has increased, rising on average by ten years since 1970. The challenge now is to sustain and enhance these achievements in a context of tight fiscal constraints in many countries combined with upward pressure on health spending from factors such as new technological advances and demographic changes. Finding policies that can make health spending more sustainable without compromising important achievements in access and quality requires effective co-operation between health and finance ministries. Sound governance and co-ordination mechanisms are therefore essential to ensure effective policy choices. Prepared by both public finance and health experts, this report provides a unique detailed overview of institutional frameworks for financing health care in OECD countries. One of the main features of this book is a comprehensive mapping of budgeting practices and governance structure in health across OECD countries.

Patient organizations today play a major role in the scientific, economic and political arenas. They have become the quasi-obliged partners of researchers, industrialists and political authorities. This book investigates this quite recent configuration by offering an in-depth exploration of three thematic issues: To what social and political stakes are patient organizations confronted as regards the transformation, production, circulation, and governance of knowledge? The involvement of patient organizations in the economic world is acknowledged as confrontational, especially when it comes to relationships with the pharmaceutical industry. To what extent do opposition, "instrumentalization", or cooperation constitute relevant models if we are to account for the multifaceted relationships between patient organizations and economic actors? Patient organisations are the structuring actors of networks, coalitions, and collectives throughout Europe. What social and political concerns arise from these particular forms of collective action on a national or European scale? Each question is first examined through an academic and grey literature review. Then the emerging topics and critical issues are identified and discussed, drawing upon exchanges of experiences, viewpoints, and reflections between actors involved in patients' and users' movements and social scientists. The dynamics of patient organizations in Europe raises a number of questions of interest for all actors from the health system, as well as for social scientists. This book intends to contribute to the reflection on further research agenda setting and policy-making.
